What We Provide

We provide residential home care for individuals with intellectual and developmental disabilities. We ensure that they are integrated into the community and are able to enjoy life in the best way possible. We seek our professional doctors, therapists, and programs that are willing to work with those we serve and not brush them aside because of their disabilities.

We care for individuals with behavioral issues, implement their behavior plans, and utilize Safetycare procedure to minimize behavioral outcomes. We

Requirements

  • All of our residents must be approved for the COMP waiver.

  • Potential residents much be approved for CRA services.

  • Individuals with behaviors requiring additional staffing will also need to be approved for ARS services before entering the home.

  • All residents much ambulate independently or without major equipment, i.e. wheelchair

  • We currently do not accept those with severe medical concerns, i.e. feeding tube, deep wound care, etc. *please reach out if unsure.

Day to Day

Our residents are encouraged to keep a schedule that is beneficial to their health and wellness, medication regimens, and community integration. We assist them with ADLs if needed and ensure that they have access to the places and things that are important to them. We like those we serve to know that they are being heard and that we are considering all aspects of their needs: behavioral, medication, physical, and mental.

If the individual already already been approved for COMP waiver services, the support coordinator will be able to start the process of seeing if Grace Healthcare Services will be a good fit. Be sure to pass our name on to them and have them call us directly at (229)531-4449. If we have a vacancy, and we are a good fit, we will definitely look forward to serving them.
